DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS 
The Houston Health Department seeks a dynamic candidate to function as a Senior Public Health Educator within the Office of Chronic Disease, supporting the Diabetes Awareness & Wellness Network (DAWN) and other chronic disease programs and initiatives. The selected candidate may work across Office of Chronic Disease program areas based on programmatic needs. 

Duties include the following: 

  • Develop and conduct health education, training, workshops, and health promotion programming, including chronic disease prevention, self-management, health literacy, and empowerment. 
  • Deliver and support evidence-based programs and curricula, including Diabetes Self-Management Education and Support (DSMES), Diabetes Prevention Program (DPP), physical activity and fitness programming, and other diabetes and chronic disease education programs. 
  • Apply appropriate behavioral theories and evidence-based models to plan, organize, implement, and evaluate community engagement, health education, physical activity, and health promotion programming in community and center based settings to improve chronic disease knowledge, attitudes, and behaviors in at-risk communities. 
  • Establish and maintain close working relationships with internal staff, community-based and private sector organizations to coordinate and perform culturally and linguistically appropriate health promotion, physical activity, and chronic disease education activities. 
  • Develop and interpret culturally and linguistically appropriate content for educational and training materials, visual aids, presentations, interactive demonstrations, virtual group classes, fitness classes, physical activity sessions, and social media settings. Content may include, but is not limited to, diabetes, cardiovascular disease, hypertension, healthy eating, physical activity, active living, tobacco cessation, and other chronic disease prevention and management topics. 
  • Perform data entry, data collection, and data management activities; prepare reports; and participate in qualitative research, quality assurance, and quality improvement activities related to chronic disease programs and evidence-based interventions. 
  • Utilize various interdisciplinary, communication, and marketing techniques to promote and disseminate actionable health and wellness information. 
  • Support positive client outcomes and quality service delivery across Dawn Program and other Office of Chronic Disease clinical and health promotion activities. 
  • Perform other duties as directed.
WORKING CONDITIONS
The position occasionally requires stooping or bending. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds or an equivalent weight) may be required. There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions.

MINIMUM REQUIREMENTS

EDUCATIONAL REQUIREMENTS:
Requires a Bachelor's degree in Education, Behavioral Science, or a closely related field such as Public Health.

EXPERIENCE REQUIREMENTS:
Two (2) years of experience in Public Health, Education or a closely related field are required.

A Master's degree in Education, Behavioral Science, or closely related field such as Public Health may be substituted for the above requirements.

Directly related professional experience may be substituted for the education requirement on a year-for-year basis.

LICENSE REQUIREMENTS
Valid Texas Driver’s License and compliance with the City of Houston's policy on driving (AP 2-2).

PREFERENCES

**Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6**

  • Experience in fitness, physical activity programming, or certification as a Certified Personal Trainer (CPT) or fitness instructor preferred. 
  • Directly related professional experience or training in diabetes, chronic disease education, or evidence-based health promotion programs preferred. 
  • Preference for applicants who are bilingual in English, Spanish, and/or Vietnamese based on community needs. 
  • Experience using Microsoft Office products, including Word, Excel, PowerPoint, and Outlook. • Experience delivering one-on-one health education, group classes, presentations, or communitybased health promotion activities preferred.

SELECTION/SKILLS TEST REQUIREMENTS
None.  However, the department may administer a skill assessment evaluation during application review and/or interview.

Pay Grade 18
 
SALARY INFORMATION    SPECIAL FUNDS
Grants & Special funded positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
